The Registered Nurse (RN) will conduct in-person assessments, care management, and wellness checks for individuals who were previously homeless and are managing physical health, mental health, and substance use disorders. Services are delivered in diverse settings, including hospitals, residential care facilities, congregate living environments, and other community locations.
